As the New Year commences we would like to invite you to the official launch of our newest healthcare services for 2015, the Liaison Team based at Chesterfield Royal Hospital. This launch follows the success of a similar team commissioned for the Royal Derby Hospital in Derby during 2013. The new service has been established by merging the former Mental Health Liaison & Self Harm Team, Older Adults Mental Health Liaison team and the Hospital Alcohol and Drugs Liaison Team (HADLT) to create a 24/7 rapid assessment and signposting service.
Hospital staff are now able to call upon our Liaison Team to offer comprehensive advice and support to patients with mental health needs or drug and alcohol issues aged 17 and over attending the Emergency Department and acute inpatient wards at Chesterfield Royal Hospital. The Liaison team provides a rapid response following referral, with a high quality intervention, assessment and, wherever practicable, discharge.
The official launch event, which is open to any interested individuals, will take place on Thursday 29 January, 10.30am – 12.30pm at the Old Lecture Theatre, Chesterfield Royal Hospital. Refreshments will be provided. To confirm your attendance, please email email@example.com or call 01246 512572 by Friday 16 January.
The Liaison Team, staffed by Derbyshire Healthcare but based at Chesterfield Royal Hospital, is an example of the kind of collborative, smarter working that is being developed in north Derbyshire through the 21st Century #JoinedUpCare programme. View a short YouTube film made by the 21C #JoinedUpCare team about the new service.
