To ensure that the people who use our healthcare services have the best possible experience, we have introduced a new quick and easy Friends and Family Test survey to capture individual feedback about what is working well and where improvements need to be made.
"How likely are you to recommend our service to friends and family if they needed similar care or treatment?"
Once you are discharged from one of our services, you will be asked the above question. Using the online survey, you will be invited to respond by choosing one of six options, ranging from "extremely likely" to "extremely unlikely". You will also have the opportunity to explain why you have given your answer.
Response to the Friends and Family Test is voluntary but we do wish to encourage you to participate. All responses are completely anonymous, but they will allow us to identify any common themes or experiences, which we can then look to address in order to improve our services and the experiences people have when in our care.

If you have any specific needs or requirements and need support to take part in the survey, please email firstname.lastname@example.org or call that Patient Experience team on 01332 623751.
You can download a print version by clicking on the online survey. Select the service you have been receiving, then select a team from the drop-down list. In the far bottom right corner of the screen, a print icon will appear (NB you may need to scroll across to the right to see it). Click on this icon and the print version will begin to download.
Don't have access to a computer? Please ask a member of staff to download the print version for you.
After completing the printable survey, please pass this on to your clinician who will ensure your feedback is submitted and enter onto the on-line Friends and Family Test form.
