Children in Care nurses: Information for young people


Our Children in Care nurses are qualified nurses who offer confidential support and information if you have worries or problems.  We are someone to talk to who will listen.  We also carry out your review health assessments.  You have the right to speak to a health professional in confidence. The only time when we do not keep confidentiality is when you are hurt or in danger.

• pregnancy testing

• sexual health

Why do I need a review health assessment?

Children and young people need to be as healthy as possible. To do this a review health assessment is completed by a nurse every twelve months.

What is a review health assessment?

The assessment is a discussion about your health and you will be given advice about health issues and leading a healthy lifestyle. We need this information so that you are looked after in the right way. After this we can then write the health plan with you.

Where will my health assessment take place?

The assessment will take place at Sinfin Health Centre.

What happens after the review health assessment?

The nurse writes up a health plan to ensure all your health needs are identified and how they will be met and who will do them.

The health plan

The health plan will be discussed at your LAC review.
It can also be reviewed at any time if your health needs change.
The plan is shared with your carer and other professionals involved in keeping you healthy.

The plan is only shared with people who need to know.
